     A pre-cut scale is included in each requested Hair Analysis Sample Kit.   

     If rather than requesting a kit through the online order form or by email

     you will be printing the request forms and scale from your computer you

     may use the form provided below.

     The scale must be:

• printed on 110 lb. paper ("cardstock")
• cropped to measure 3" x 6" for accuracy

     Click the printer icon on the document and use your printer's menu

     (when your print dialog box opens) to select the correct size.  If your

      print menu allows, you may also choose to "print to file" and use

      a photo editing/printing program to attain the correct size.

      An accurately measured hair sample will be approximately 1 Tablespoon

      by size (but not weight) or 2.1 Grams.
